New Patient Registration

We welcome new patients who are living in areas covered by G74 or G75 postcodes

Permanent Patients

If you will be living in the area for greater than 3 months then application for patient registration to the Surgery may be made. This will allow us to provide you with a full range of NHS general medical services and request transfer of your medical records from your previous GP. Our reception staff will be happy to advise you of the process which essentially involves the following steps:

  • Please contact the practice to request a registration form which should be completed and returned to the Practice along with a copy of supporting documents of proof of permanent address  e.g. utility bill, bank statement, council tax bill AND a form of photographic ID e.g. passport or driving license (during Covid 19 pandemic we will accept return of forms via post or you can place forms in a sealed envelope in our practice post box located on the ground floor of the health centre located immediately to the right of the door as you enter the building).
  • If you have a Chronic Medical Condition e.g. Heart Disease, Diabetes, Asthma/COPD, Please book and attend a new patient medical appointment with our practice nurse.  This involves some basic health checks and gathering some more information about your health, any current medical problems and medications. Even if you are well it is an opportunity for a health check up.
